• Monthly Membership: Allow for 18 months to complete program
• First 6 weeks: Weekly visits with either the dietitian or Dr. Novick
• Remainder of 18-Month Plan: biweekly or monthly visits with dietitian or Dr. Novick
  

6-Month Program: $399/month

• For those requiring safe and rapid weight loss prior to surgery/procedure
• First 6 weeks: Weekly visits with either the dietitian or Dr. Novick
• Remainder of 6-Month Plan: biweekly or monthly visits with dietitian or Dr. Novick
• Requires a referral from your surgeon or PCP

Maintenance Program: $205/session

• Meet quarterly with Dr. Novick or dietitian for sustainable weight loss

Notes:

• Insurance is not required. We will supply you with a Superbill upon request for reimbursement from your insurance company or HSA plan.
• Weight Loss Medications can be used (or not used).
• Weight Loss Medications may be covered by your insurance company.
• Individualized food plans with compassionate care with our experienced dietitian
• We use high quality Numetra® full meal replacements during your program.
• 100% meal replacements cost an additional $150-$200/week for 12 weeks.
